Woven Health Earns Sixth Consecutive National Recognition for Patient-Centered Care

Woven Health Clinic, a leading provider integrated community healthcare, announces the repeat accreditation for the renowned National Committee for Quality Assurance (NCQA) Patient Centered Medical Home (PCMH) award. This recertification is Woven Health’s sixth year to be recognized, setting them apart from other providers and confirming their ongoing commitment to continuous quality improvement and a patient-centered approach to care.

 

Originally launched in 1990, NCQA exists to improve the quality of health care. They are designed to identify medical practices that invest in a model of care and ensure continuous quality improvement as a priority. As the model of care that puts patients at the forefront of care, NCQA PCMH accredited facilities build better relationships between patients and their clinical care teams Woven Health’s award as an NCQA PMCH center underscores its dedication to providing quality integrated healthcare for its patient community.

Celebrating its 30th year of service, Woven Health serves patients in two locations by providing a wide range of comprehensive services, including health and lifestyle assessments, depression screening, primary care, disease prevention education, cancer screenings, immunizations, nutrition therapy, mental and behavioral health care, women’s health care, and referrals to other specialists and social service organizations.
