What to bring to your appointment

New patients, please bring the following to your appointment:

All medications that you are currently taking
The name, phone number and address of your preferred pharmacy
The name and phone number of your previous physician
Medical records from your previous physician — please have these faxed, emailed or mailed prior to your visit
Medical records from any emergency room visits within the last 12 months
A form of identification with your picture (driver’s license, passport, matriculate, student ID)
Proof of income
Completed new patient forms (click here to download)

Returning patients, please bring the following to your appointment:

All medications that you are currently taking
A form of identification with your picture (driver’s license, passport, matriculate, student ID)
Medical records from any emergency room visits since your last appointment

Cancellation and reschedule policy

We strive to be flexible and accessible for our patients, and recognize that sometimes patients will need to reschedule or cancel appointments. In order to best serve all our patients, scheduled appointments must be cancelled or rescheduled no less than 48 hours prior to the appointment during business hours.
